When the key comes out at ON or ACC, that interlock has failed. Whatever else is true, a designed safety mechanism is no longer doing its job.
Why It Actually Matters
Three consequences, in order of how likely they are to bite you.
1. You will eventually walk away from a running car. This is the common one and it is not hypothetical. The Four Realistic Causes
Running roughly from most to least common on the vehicles we see in this area.
1. A worn cylinder with a worn retaining mechanism
The most common by a wide margin, and it is a mileage-and-years problem rather than an event.
Every start cycle drags the retaining pin across the cylinder body. Over ten or fifteen years and twenty thousand cycles, both the pin and the groove it rides in wear down. Eventually the groove is shallow enough, or the pin short enough, that the key can be pulled past it with normal hand pressure.
The tell is gradual onset. It started as "the key comes out if I really tug it" and became "the key comes out." Nothing happened; it just wore.
2. A worn key
The key itself can be the whole story, and this is the cheap outcome worth checking first.
A key worn down over a decade sits slightly differently in the cylinder than a correctly cut one. On some platforms it does not fully engage the mechanism that holds it, so it slips out of positions where a good key would be trapped.
The test costs nothing: try the spare, especially one that has lived in a drawer. If the spare stays put and the daily key comes out, you have a key problem rather than a cylinder problem, and the fix is a new key cut to the vehicle's factory code — not copied from the worn key, which just reproduces the wear.

4. Debris or a foreign object in the cylinder

Two Things to Check Before You Call
Both are free and both narrow the diagnosis meaningfully.
Try every key you own. Including the spare in the kitchen drawer, the one at your parents' house, the valet key in the glovebox. If any of them stays put where the daily key does not, this is a key wear issue and the repair is at the cheap end.
Note exactly which positions release the key. There is a difference between "it comes out at ACC" and "it comes out at ON with the engine running" and "it comes out in any position including START." One thing not to do: do not spray lubricant, graphite or penetrating oil into the ignition to see if it helps. The engine will not stop because the key came out. The car does not become undriveable.
